What is US Core?

US Core is the HL7 FHIR implementation guide that defines the minimum profiles and value sets US healthcare systems use for interoperability, including USCDI conformance.

In context

US Core constrains base FHIR resources (Patient, Observation, Condition, MedicationRequest, and many others) to the structure US payers and providers exchange. Each US Core profile pins required elements, value sets, and mappings to USCDI data classes.

US Core is what most US health systems implement when they expose FHIR. App developers building for the US market typically validate against US Core profiles to ensure their data is exchangeable with the surrounding ecosystem.

How Fire Arrow handles it

Fire Arrow stores standard FHIR R4 with profile validation. US Core conformance is a profile-validation configuration. Applications that produce US Core-conformant resources can validate them against the published profiles before they enter the store.
