What is ISiK profile?
ISiK (Informationstechnische Systeme im Krankenhaus) is the FHIR profile family the gematik defines for the German hospital information system ecosystem and its data-exchange partners.
In context
ISiK is rolled out in stages. Each stage adds clinical domains and participants in the exchange. Hospital information systems certified for an ISiK stage have to support reading and writing the resources that stage covers; downstream systems (DiGAs, regional health platforms) increasingly need to interoperate with ISiK-conformant data.
From an integration standpoint ISiK is a profile validation problem: the resources are standard FHIR R4 with German-specific constraints. Applications that store FHIR R4 with profile validation can produce ISiK-conformant outputs without a schema migration.
How Fire Arrow handles it
Fire Arrow stores standard FHIR R4 with profile validation. ISiK conformance is a profile configuration; the data layer does not change. Specific ISiK module conformance is a per-deployment integration concern.
