What is FHIR R5?
FHIR R5 is the fifth major release of the HL7 FHIR specification, published in 2023, with normative additions to several resource types and changes to terminology and search behaviour.
In context
R5 introduced new resource types (ImagingSelection, GenomicStudy, others), promoted several existing types to normative status, refined search parameter semantics, and added SubscriptionTopic / SubscriptionStatus for the topic-based subscription model.
Most production deployments still run R4 because it is the version most regulators and exchange partners reference. R5 adoption is gradual and typically driven by specific use cases that benefit from R5-only features.
